Former This Morning host Phillip Schofield has shared a series of his final photos with his mum, Pat, after he announced that she passed away today following an illness over the weekend

Phillip Schofield shared his final photo with his mum Pat, taken before she sadly passed away today.

In a series of sweet photos, posted on the 62-year-old's Instagram Story, Phillip has shared some of their final memories together, including a photo of him with his arm around her iin the countryside and another of her smiling at the theatre.

The final photo he posted on his story showed him and his mum beaming from ear to ear as they took a photo together infront of the Blackpool Tower.

The former This Morning star has regularly shared updates of his mum, and announced his sad news on Instagram. He wrote: "Today, at 3pm and after a heartbreaking weekend, my magnificent mum decided it was time to go to her beloved Brian. Over three days, we held her, loved her, hugged her and said everything that needed to be said. She was calm, peaceful, beautiful and dignified. I will forever be proud and privileged that she was my mum."

Phillip added: "Our eternal thanks to the paramedics, Treliske A&E and Critical Care team who have been, in the past and this weekend, my mum's and our angels. Your love and care has been our saviour."

The TV star didn't confirm his mother's cause of death, but he has discussed his mum's health condition while filming Channel 5 series Cast Away, which aired last month.

The presenter confessed his shock at the news that both his mum and brother were diagnosed with the same serious health condition - Type 1 diabetes. In a chat with Health Awareness, Phillip disclosed: "My brother and mother are both Type 1 diabetics and have been for the last 25 years, so it is something that as a family we are very used to.

"If either of them have a hypo (a Hypoglycaemic episode which occurs when blood glucose levels drop, usually below 4 mmol/L), we can recognise the signs and we know what to do. My Mum goes particularly bonkers when she goes low, so when she starts talking nonsense we know what's happening."

He continued: "My brother was first to be diagnosed and he had an unquenchable thirst for fizzy drink, which was very unlike him. That continued for a few days, he was acting completely out of character. So we took him to the doctors and he was very quickly diagnosed. Then six months later my mum was diagnosed in the same way."

Pat was rushed to hospital last June following Phillip's "unwise but not illegal" affair scandal, after he moved home to Cornwall to be with his family during the stressful time.

A source told MailOnline at the time: "It's a relief for everyone that Pat is home, it has been a terribly torrid time for her. She has been under so much stress lately. It is a lot for anyone, let alone someone of her age. Phillip is playing the dutiful son and is planning to spend time in Cornwall caring for her."
