A new initiative is helping eligible families receive fresh kosher groceries and prepared meals through the newly launched NY State SCN Food Program. The Social Care Network (SCN) Food Program connects eligible Medicaid members with free, medically tailored meals and groceries.
The program operates in partnership with Nutrition@The Market Place and Nutrition@The House of Glatt, providing medically tailored food support based on nutritional needs and household size.
Families approved for grocery assistance through Nutrition @ The Market Place can receive kosher proteins, fresh vegetables and fruits, dairy products, fish, meat, healthy starches, and pantry staples. Participants approved for prepared meal support can receive fresh ready-to-eat kosher meals through Nutrition @ The House of Glatt in partnership with Kingston Pizza.
The Market Place meat department operates under CHK certification, while the fish and bakery departments are certified by OK Kosher. House of Glatt and Kingston Pizza are under CHK certification.
The program is designed to assist pregnant and postpartum mothers, individuals recovering from illness or miscarriage, families managing chronic health conditions, and households caring for children with special needs.
Applicants may qualify if enrolled in a Medicaid Managed Care Plan such as Fidelis Care, UnitedHealthcare, or Healthfirst, and meet the program’s medical eligibility requirements.
Organizers say the initiative is aimed at improving access to nutritious kosher food while supporting community wellness in Crown Heights.
